    1. Total body weight (TBW)
    2. Ideal Body Weight (IBW)
    3. Adjusted body weight (ABW)
  1. General
    1. Consult infectious disease specialists as needed
    2. Many Antibiotics require no dose adjustment in Obesity (e.g. Doxycycline)
  2. Beta Lactam Antibiotics (e.g. Penicillins, Cephalosporins)
    1. Dose at upper limit of normal (e.g. Cephalexin 1000 mg every 6 hours)
  3. Clindamycin
    1. Use higher doses (e.g. 900 mg orally every 8 hours) in Skin and Soft Tissue Infections
  4. Trimethoprim/Sulfamethoxazole (Bactrim, Septra)
    1. Use 2 tabs twice daily
